The West Lafayette Police Department can conduct a criminal/pre-employment character history ($3.00 fee required). This may be obtained during normal business hours.

Required:

• Two forms of identification (at least one photo identification) are required.
• U.S. Mail requests must be accompanied by a notarized statement (attesting to the fact that the notary verified the identification of the requestor)
• a prepaid check or money order
• a return stamped and self addressed envelope (we are unable to fax copies of character checks)
• Full name
• Date of birth
• Social security number 

Employers conducting history checks on employees or applicants must provide a document on company letterhead that has been signed by the individual whos history is being checked and the signature must be witnessed. Full name, date, and social security number is required. U.S. Mail requests must be accompanied by the same required document, a prepaid check or money order, and a return stamped and self addressed envelope(we are unable to fax copies of character checks).

Government agencies are exempt from the $3.00 fee.
